To make the most of your consultation, please complete the appropriate checklists below as best as you can.  It's ok to not have 'all' the answers - we will help you resolve your questions and prepare a detailed plan together.

Here's a Guide to what to bring....

If you are coming in to prepare a Special Needs Plan, please complete Worksheets #1 and #5.

If you are coming in to review a Special Needs Plan that you have already established, please complete Worksheets #3 and #5.
